*** FINAL FORMS Student Online Registration **** |
Jan 7, 2021 2:37 PM |
We are very excited to announce that the Tacoma Public Schools Athletic Department has partnered with FinalForms, an online forms and data management service. FinalForms allows you to complete and sign athletic participation forms for your students. The most exciting news is that FinalForms saves data from season-to-season and year-to-year, meaning that you will never need to enter the same information twice! FinalForms also pre-populates information wherever possible, for each of your students, saving you time. You may review your data at any time to verify it is current. You will be required to sign your forms once per year and after any update. This is the registration process for ALL ATHLETES, regardless of Running Start, Homeschool, Private School or Alternative School enrollment. However, you will need to complete the relevant Running Start, Homeschool/ Private School/Alternative forms within FinalForms online registration. SAMI/SOTA/IDEA Students, you need to contact the Stadium AD prior to online registration. This does not roll over in the FinalForms system and must be done annually. In order to register you must confirm your account using the link provided in the FinalForms email. If you did not receive an email from FinalForms or the link has expired, please contact the Stadium Athletic Director: MHayes@Tacoma.K12.WA.US

FinalForms Troubleshooting |
Jan 18, 2021 1:05 PM |
Stadium Athletics receives emails from students and parents needing help with various issues related to logging into their FinalForms accounts. Unfortunately, we do not have a list of usernames and passwords or a way to reset passwords. However, here is a list of what may be causing issues and possible resolutions: - TPS (eSP) & FinalForms are not communicating.
- Tacoma Public School students must use their school email for FinalForms. If you (student) have changed your HAC contact email to a personal address (Gmail, Yahoo, etc.), you need to change it back to your assigned school email address.
- The student has never logged into FinalForms.
- The student will need to confirm their account. Students need to login to their Office 365 account (same username and password needed to login to a school computer). Once confirmed, athletes should go to Outlook email where they will find their FinalForms Account confirmation link. If they cannot find the email, the Athletic Director can resend a link to confirm their account.
- Parent has never logged into FinalForms.
- They need to confirm their account. Parents should search their email for a FinalForms confirmation email. If parents cannot locate their confirmation email, they will need to contact the building Athletic Director to confirm the email address Tacoma Public Schools has is correct. We can then resend a confirmation email.
- Parent or athlete have logged in but forgot their password.
- Please use the “Forgot your Password” feature on the FinalForms login page.
If the parent does not have a valid email in eSchoolPlus (HAC) there will not be an account for the student. Parents need to update their email with the school’s Registrar and the Athletic Director. Homeschooled. Private School, Alternative Education students will need to have an account created for them. Please contact the Athletic Department: StadiumAthletics@Tacoma.K12.WA.US
